Autologous Donations

Physicians who wish to order autologous donations for their patients may do so by completing LifeShare’s Autologous Donation Form

The patient must present the completed form at the LifeShare donation center where the blood collection is to take place.

Directed Donations

If your patient cannot give blood for himself, directed donors may be selected to give blood for the patient’s use.  Directed donors must have the same blood type and must meet the same donor requirements as a whole blood donor.  A consent form is signed by the donor releasing their unit of blood to be used by the patient.
