Uniform Donor History and ID Verification (UDHQ)
UDHQ is the FDA recommended process for screening blood donors.  The process consists of a Uniform Donor History Questionnaire, a request for identification and a new important information handout.

We are implementing UDHQ to follow FDA recommendations, to improve the consistency and quality of information collected from donors nationwide, and to better ensure the safety of the blood supply.

The Important Information sheet has changed.  Donors should know the overview of the educational information, reasons they should NOT attempt to donate, definitions that are provided to help donors give more accurate information and the medication deferral list.

The donor registration process changed.  The donor is required to provide identification prior to donating, preferably a drivers license. Other forms of identification will be accepted. A complete list of optional identification will be provided on a registration poster.
