- Date of Birth
- Do you wish your blood to not be used for confidential reasons?
- Telephone number you can be reached at
Symptoms (fever, sore throat, congestion, diarrhea, vomiting, headache…)
- Date your symptoms began
- Current medications (including over-the counter)
- Are you currently taking any antibiotics?
- Have you seen a doctor?
